Past studies have not detected consistent improvement in ventricular function (VFxn) following initiation of cardiac resynchronization therapy (CRT) in Fontan patients. However, these studies used qualitative assessments of VFxn and/or quantitative assessments of VFxn that rely upon anatomic and/or geometric assumptions that may not be valid in patients with single ventricles. To address this, we used quantitative indices of global VFxn (dP/dtic and the Tei index) that are not encumbered by the limitations associated with the indices used in previous studies of CRT in Fontan patients. Patients with Fontan physiology who had received CRT therapy from 2004 to 2019 were included in the study. They were compared to a concurrent group of Fontan patients who had received standard dual-chamber pacemakers (DCPMs). VFxn was assessed at 3 time points prior to, shortly after, and late after initiation of pacemaker therapy. Prior to initiation of pacemaker therapy, VFxn of the CRT patients tended to be worse than thatmpared to standard DCPM. Changes in VFxn correlate with concurrent changes in New York Heart Association classification.Transcatheter aortic valve implantation (TAVI) is emerging as the default strategy for older patients with severe, symptomatic, and trileaflet aortic stenosis. Increased body-mass index (BMI) is associated with a protective effect in patients undergoing percutaneous coronary intervention. We assessed whether elevated BMI was associated with a similar association in TAVI. We evaluated prospectively collected data from 634 patients who underwent TAVI at 2 centers from August 2008 to April 2019. Patients were stratified as normal weight (BMI 18.5 to 24.9 kg/m2, n = 214), overweight (25 to 29.9 kg/m2, n = 234), and obese (>30 kg/m2, n = 185). Outcomes were reported according to VARC-2 criteria. Mortality was assessed using Cox proportional hazards regression analysis (median follow-up 2 years). Kaplan-Meier analysis was used to estimate cumulative mortality. Baseline differences were seen in age (85 vs 84 vs 82, p less then 0.001), STS-PROM score (4.3 vs 3.4 vs 3.6, p less then 0.001), sex (50% vs 36% vs 55% female, p less then 0.001), clinical frailty score (p = 0.02), diabetes (21% vs 29% vs 40%, p less then 0.001), and presence of chronic obstructive pulmonary disease (COPD) (13% vs 13% vs 23%, p = 0.009). On multivariable analysis there was no mortality difference between normal and obese patients (hazard ratio [HR] 0.70, confidence interval [CI] 0.46 to 1.1 p = 0.11), however overweight patients had significantly lower mortality (HR 0.56 CI 0.38 to 0.85, p = 0.006). Variables independently associated with increased mortality were increasing age, male sex, COPD, previous balloon valvuloplasty, and higher STS-PROM. In conclusion, overweight patients have lower long-term mortality when compared with normal weight and obese patients undergoing TAVI.A deeper understanding of the processes underlying the distribution of animals in space is crucial for both basic and applied ecology. The Common buzzard (Buteo buteo) is a highly aggressive, territorial bird of prey that interacts strongly with its intra- and interspecific competitors. We propose and use random matrix theory to quantify the strength and range of repulsion as a function of the buzzard population density, thus providing a novel approach to model density dependence. As an indicator of territorial behaviour, we perform a large-scale analysis of the distribution of buzzard nests in an area of 300 square kilometres around the Teutoburger Wald, Germany, as gathered over a period of 20 years. The nearest and next-to-nearest neighbour spacing distribution between nests is compared to the two-dimensional Poisson distribution, originating from uncorrelated random variables, to the complex eigenvalues of random matrices, which are strongly correlated, and to a two-dimensional Coulomb gas interpolating between these two. A one-parameter fit to a time-moving average reveals a significant increase of repulsion between neighbouring nests, as a function of the observed increase in absolute population density over the monitored period of time, thereby proving an unexpected yet simple model for density-dependent spacing of predator territories. A similar effect is obtained for next-to-nearest neighbours, albeit with weaker repulsion, indicating a short-range interaction. Our results show that random matrix theory might be useful in the context of population ecology.Ulcerative colitis (UC) often occurs accompanied by colonic leakage and flora imbalance, resulting in secondary liver injury (SLI). SLI, in turn, aggravates UC, so the treatment of UC should not ignore it. β-patchoulene (β-PAE), a tricyclic sesquiterpene isolated from Pogostemon cablin, has been reported to exert a protective effect in gastrointestinal disease in our previous studies. However, its protection against UC and SLI remains unknown. Here we explored the protective effect and underlying mechanism of β-PAE against dextran sulfate sodium-induced UC and SLI in mice. The results indicated that β-PAE significantly reduced disease activity index, splenic index and attenuated the shortening of colonic length in UC mice. It alleviated colonic pathological changes and apoptosis through protecting tight junctions, reducing neutrophil aggregation, and inhibiting the release of pro-inflammatory cytokines and adhesion molecules. These effects of β-PAE were associated with the inhibition of TLR4/MyD88/NF-κB and ROCK1/MLC2 signalling pathway. UC-induced colonic leakage caused abnormally high LPS levels to result in SLI, and β-PAE markedly inhibited it. β-PAE simultaneously ameliorated SLI with reduced biomarker levels of endotoxin exposure and hepatic inflammation. High levels of LPS were also associated with flora imbalance in UC mice. However, β-PAE restored the diversity of gut microbiota and altered the relative abundance of characteristic flora of UC mice.
